Ready to Apply for Medicaid?

To apply for UPMC for You, you must qualify for Medical Assistance (Medicaid) through the state of Pennsylvania. The Department of Human Services (DHS) will determine whether you qualify for Medicaid and what benefits you and/or your family members will receive.

Need to apply for Medicaid (Medical Assistance)? You have options!

Use COMPASS or your County Assistance Office:

  1. Online: You can use the COMPASS website to apply for Medicaid and many other services that can help you.
  2. By phone: You can call the Consumer Service Center at 1-866-550-4355 to apply for health care coverage.
  3. In person: You can contact your local County Assistance Office (CAO).
  4. On paper: You can download and complete an application, then send it to your local CAO. If you need help completing the application, a CAO staff member can help you.

 

How to choose UPMC for You as your Medicaid managed care organization (MCO)

There are several ways to do it:

  1. Online: Visit enrollnow.net/enroll to enroll.

  2. By app: Download the PA Enrollment Services app.
  3. By phone: Call PA Enrollment Services at 1-800-440-3989 to enroll in UPMC for You. TTY users should call 1-800-618-4225. Help is available Monday through Friday from 8 a.m. to 6 p.m. Customer service is offered in multiple languages.
  4. By mail: If you are eligible, you will be mailed an enrollment form. Fill out the form by the date listed and mail it to the address below.

  • Monday, Tuesday, Thursday, and Friday from 8 a.m. to 6 p.m.
  • Wednesday from 8 a.m. to 8 p.m.

Free Transportation Benefit Limitations

UPMC for You’s transportation benefit provides nonmedical, fixed route rides and is available to members who live in the Northeast or Southeast Physical HealthChoices zones only. The counties included in these zones are Bradford, Bucks, Carbon, Centre, Chester, Clinton, Columbia, Delaware, Juniata, Lackawanna, Luzerne, Lycoming, Mifflin, Monroe, Montgomery, Montour, Northumberland, Philadelphia, Pike, Schuylkill, Snyder, Sullivan, Susquehanna, Tioga, Union, Wayne, and Wyoming. The benefit is available at no extra cost. Mobile and paper ticketing options vary by county. If you are a UPMC for You member living in the Northeast or Southeast Physical HealthChoices zones and have questions about how to use the benefit, please call the UPMC for You Transportation Unit at 1-833-779-6827 (TTY: 711).

The Transportation Unit is available from Monday through Friday from 8 a.m. to 5 p.m. and is closed for New Year's Day, Easter, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.
