Connect with resources to help you live healthier

We are here to improve your quality of life.

Living a healthy life depends on many things. Based on where you live, you might not have access to things that can help you live a healthy life.

  • Some areas do not have safe housing or transportation.
  • If you do not live near a healthy grocery store, it can be hard to eat well.
  • Some communities do not have safe places to exercise and play.
  • Some areas do not offer good schools (education) for children.
  • It might be hard to find a job or locate job training centers in certain areas. This could make it harder to be financially stable.
  • Some areas have unhealthy air and water.

We’re here to help you

Connect with a resource navigator

If you are facing challenges to living a healthy life, call the UPMC for You Resource Navigation Hub at 1-833-223-5753 (TTY: 711). Help is available Monday through Friday from 9 a.m. to 5 p.m. A resource navigator can connect you to help with these and other needs*:

  • Affordable groceries
  • Help paying for utilities
  • Help finding stable housing
  • Clothing
  • Childcare
  • Transportation
  • Job support

Our navigators can also connect you to PA 211 navigators dedicated to assisting UPMC for You members. PA 211 helps individuals access information and referrals to essential community resources. The navigators at PA 211 will listen, identify underlying problems, and connect you to the resources and services you need within your community.

*UPMC for You’s navigators can help members apply to the Low-Income Home Energy Assistance Program (LIHEAP), a federal program that helps individuals and families with energy costs, and/or the Supplemental Nutrition Assistance Program (SNAP), a federal program that provides assistance to low-income individuals and families to purchase food. Submitting a LIHEAP or SNAP application does not guarantee enrollment in the programs. Program eligibility is determined by the PA Department of Human Services (DHS). UPMC for You can also help members connect to resources to find stable housing. UPMC for You does not pay for members’ grocery, utility, or housing bills.
