Intellectual Disabilities/Autism Services

UPMC for You combines covered benefits with enhanced targeted services to support UPMC for You members who have an intellectual disability or autism (ID/A) and their families.

UPMC for You is committed to making sure that members have access to the covered benefits listed below.

 

Other services for members with ID/A

As a UPMC for You member, you have access to our Special Needs Unit (SNU). This is a team of nurse and social work care managers. They can help you get the services you need and coordinate your care with different providers and programs. They can also help you transition from pediatric to adult services and connect you and your caregivers with community resources.

Access care. The main purpose of the SNU is to make sure you get the primary and specialty care you need. The SNU gives you a single point of contact to help you understand your insurance, navigate the health care system, choose a primary care provider (PCP), and find any specialists you might need. Our SNU care managers are trained to use LifeCourse tools to develop a care plan that will help you achieve your health care goals. They can also find other services and supports for which you may be eligible, then connect you with community organizations and state or county resources.

Care coordination. Our care managers can help coordinate your care across state, county, and local programs They also work with ID/A providers and coordinators to make sure you get the care you need. Our care managers can also help when you are transitioning from a hospital or temporary medical setting to care at home.

Transition services. Care managers can help you transition from pediatric to adult care. They can work with you, your family, and your caregiver to find helpful resources. They will stay involved during the transition process.
